The CNA - Outpatient Hospice provides essential patient care services under the guidance of a registered nurse in various home settings such as residential homes and assisted living facilities. Responsibilities include assisting with daily living activities, monitoring patient status, documenting care, and reinforcing patient and family education. The role emphasizes maintaining patient comfort and safety while delivering compassionate care to individuals in need.

Summary:
Performs specific patient care procedures (such as basic wound care and catheter maintenance) and tasks under the direction of the primary registered nurse or other licensed clinician. All procedures, duties, and tasks are to be completed under the patients care plan as ordered in conjunction with the physician. Patient care primary setting is in the patient home which can include single family residence, Assisted Living Facilities (ALF), or Nursing Home. Patient level of care can range from skilled Home Healthcare, Palliative, and/or Hospice.

*1 year of direct patient care in any clinical setting, or in-home health/hospice is preferred.
