MUST HAVE: Homecare Homebase & Oasis Experience and 2 years as an LPN LOCAL PAY: $53 base / $79.50 OT Start Dates: 5/5 13-week contract Hours: 40 hours Expected to work 40 hours per week. Must have OASIS experience Oasis E - after Jan 2023 experience is required No no more than 1 year gap All documentation due same day Homecare Homebase (Productivity points, minimum average26-28) That could mean 3 people or 7 people About 20-25 cases per week. 5-7points per day Absolute must - true home health experience, Oasis documentation Facility Name: AccentCare Petaluma Location: Main office -1301 Redwood Way, Suite 240 | Petaluma, CA 94954 Facility size: Varies - home-based care Marin County Sonoma County Scrub Color Requirement: any color PPE: TBD Parking: TBD Pay Cycle: Monday-Sunday EMR: Must have OASIS experience Case types: LVN Home Health Orientation: About 2-3 days of orientation. Modules, get intro to the team Other Details: Once a week they have to go into the office (Usually Tue, Wed and Thur). Mandatory in person Hours: Branches are open 8am - 5pm, they may have patients who want to be seen before or after. Homecare Homebase MUST be completed at the patients home. Absolute must - true home health experience, Oasis documentation Mileage will be reimbursed at .70/per mile
